• Please submit your request through the comments section for an outreach team to conduct a site visit. Please include the following information, if known: 
    • Identify the last known location, as well as other locations the homeless individual or family have been known to congregate. 
    • Identify whether it is a family or individual household. 
    • If the homeless household is an individual, please identify the sex of the individual. 
    • For family households, identify the number of people to include how many are adults and how many are children.
    • If known, please provide the name of the homeless person(s).
    • Identify if the homeless person uses a wheelchair or other assistive devices
